DVACO Preferred Partner Networks are carefully selected organizations that demonstrate commitment to high-value care and collaboration with our patients, providers, and partners. Our networks are regularly reviewed for quality, cost efficiency, and satisfaction outcomes by industry experts. 

Preferred Clinic-Based Outpatient Therapy Network

Our preferred clinic-based outpatient therapy network serves patients in the traditional community outpatient therapy setting. Patients served in this setting have the ability to leave their home to receive rehabilitative therapy treatment and may have access to more specialized equipment and resources. 

Preferred In-Home Outpatient Therapy Network

Our preferred in-home outpatient therapy network serves patients who need in-home physical, occupational and/or speech rehabilitation therapy. These patients are either unable, or it is too difficult for them to get out of their home to go to an outpatient therapy gym. Care plans are tailored to the home environment, allowing exercises to be practiced in real life settings. If patients have skilled nursing needs in addition to in-home therapy needs, then please contact one of our preferred home care providers listed below. 

Preferred Home Health Organization Network

Our Preferred Home Health Network serves patients confined to their home who are in need of short-term nursing, physical therapy, occupational therapy, and/or speech therapy care. 

Preferred Skilled Nursing Facility Network

Our Preferred Network list of skilled nursing facilities (SNFs) is developed to serve patients who need short-term nursing, physical therapy, occupational therapy, and/or speech therapy to help prepare them to successfully return to their home.
